IPP Therapy Dog Team Application

This is the primary form to apply to become a therapy dog team with Invisible Paw Prints. If there is an IPP examiner in your area, you will be advised of the next testing date, time, and venue. If you are an existing therapy dog team with another organization, you do not have to test but are required to fill out the online application form and then upload your current therapy dog certificate and/or ID card in this application. If there is not an IPP examiner in your area (typically within 2 hours from Charlotte, NC) please upload your AKC Canine Good Citizen (CGC) certificate, appendixes K and O in the members guidelines in this application.     I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THERAPY DOG EVENTs, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity. In consideration of my application and permitting me to participate in this activity, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: (A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: Invisible Paw Prints, Inc. (IPP) and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers; (B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise. I acknowledge that IPP and their directors, officers, volunteers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf. I acknowledge that this activity may involve a test of a person's physical and dog handling skills and carries with it the potential for damage and/or injury. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, lack of hydration, and actions of other people including, but not limited to, participants, volunteers, monitors, and/or producers of the activity. These risks are not only inherent to participants, but are also present for volunteers. I understand while participating in this activity, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and assigns. The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL. (Print signature below):
    EXAMINER APPROACHING APPLICANT & CANINE 1. Handler is friendly and courteous. 2. Canine is calm and friendly 3. Handler is dressed appropriately and canine is clean and well groomed. HANDLER/CANINE TEST 1. Canine walks with handler with no excessive pulling. 2. Canine makes left and right turns. 3. Canine sits or stands for 15 seconds with handler. 4. Canine sits or stands calmly with another canine passing (5' apart). 5. Handler understood all instructions from examiner. 6. Handler praises canine. 7. Handler adhered to all IPP guidelines. APPROACHING CANINE WITH CANINE 1. Examiner approaches applicant with canine. Canine remains calm. 2. If available, examiner uses crutches, wheelchair, or walker near canine; canine remains calm. 3. Examiner walks by canine at a brisk walk; canine remains calm. 4. Examiner walks by canine slowly and erratic like senior; canine remains calm. 5. Small canine (14 pounds and under) is calm when held in examiners hands and lap (doesn't get overexcited to get back to owner). Must not show separation anxiety from handler. CANINE APPROACHING STRANGER 1. Canine approaches stranger, canine acts calmly. 2. Canine approaches stranger and stranger touches canine; canine remains calm or playful (no fear or nervousness). Any signs of aggression or excessive barking is immediate failure and remove team from facility. EXAMINER CAN ASK ANY QUESTIONS PERTAINING TO THE OPEN BOOK TEST.
